La ley del tabaco es el mayor avance en salud pública de los últimos 15 años. No se trata de un comentario triunfalista. Es la pura verdad. La identificación del humo ambiental de tabaco (HAT) como carcinógeno para los humanos, realizada por la Agencia Internacional de Investigación sobre el Cáncer (IARC) de la Organización Mundial de la Salud (OMS)1 constituye su fundamento cientÃfico y ético. La puesta en marcha de la ley no ha estado exenta de dificultades, más relacionadas con la feroz resistencia de la industria tabacalera que con las propias debilidades de la ley. Sin embargo, estamos en condiciones de afirmar que esa ley ha alcanzado el 80% de sus objetivos sin que se hayan producido conflictos, pérdidas económicas en el conjunto del sector de la hostelerÃa ni otras disfunciones sociales.
En España, el tabaquismo continúa siendo la causa aislada más importante de morbilidad y mortalidad prevenibles2 . A pesar de su importancia actual, durante más de cuatro siglos el uso del tabaco constituyó una conducta minoritaria en nuestro paÃs. Sin embargo, entre 1940 y 1980, el consumo aumentó rápidamente hasta alcanzar al 65% de los varones y al 16% de las mujeres. A pesar de la posterior reducción y estabilización, España continúa siendo uno de los paÃses de la Unión Europea (UE) con mayor prevalencia de tabaquismo, situándose ésta por encima de la media de los 15 paÃses que han constituido la UE hasta el 1 de mayo del año 20043,4 . Mientras que la mayorÃa de los 25 paÃses que actualmente la integran han disminuido su prevalencia un 20-25% (o más) en los años 1985-2003, España sólo ha reducido la prevalencia del tabaquismo en la población general un 10,5%5 .
